HEART AND LUNGS TRANSPLANT PROCESS

HEART TRANSPLANT PROCEDURE

Heart transplantation is a life-saving treatment option for a person suffering from various end-stage heart diseases. Organ transplantation is performed for patients who have received all other medical therapies, device treatment and surgical intervention options yet, have not recovered. In such cases, heart transplantation is the last resort recommended by doctors.
The cardiac units and operation theatres are well equipped and care has been taken to make it patient centric. It has been made keeping in mind the comfort of our patients and to provide them with the best care and service possible.
A heart transplant surgery is suggested in severe cases of heart failure. It is also considered when the patient does not respond to other medications or medical procedures. The surgery is performed under general anesthesia.

A list of procedures is mentioned below to perform the heart transplantation.

During the procedure, the surgeon makes an incision in the center of the chest and opens the rib cage by performing sternotomy. A heart-lung bypass machine is connected which makes the blood circulate around your body. Your heart is then removed leaving behind a section of right and left atria. Then, the new heart is connected to the aorta and other main arteries. Once the new heart is able to pump the blood and is stable, then the heart-lung machine is removed. Finally, the incision is closed with suture and then covered with sterile dressing. What Is a LVAD?

A left ventricular assist device (LVAD) is a pump that we use for patients who have reached end-stage heart failure. We surgically implant the LVAD, a battery-operated, mechanical pump, which then helps the left ventricle (main pumping chamber of the heart) pump blood to the rest of the body. LVADs can be used as:

  • Bridge-to-transplant therapy: This is a life-saving therapy for patients awaiting a heart transplant. Patients use the LVAD until a heart becomes available. In some cases, the LVAD is able to restore the failing heart, eliminating the need for a transplant. Learn more about heart transplant.
  • Destination therapy: Some patients are not candidates for heart transplants. In this case, patients can receive long-term treatment using an LVAD, which can prolong and improve patients' lives.

LVAD implantation: What to expect

We will first determine what type of LVAD patient you are: either bridge-to-transplant or destination therapy. This designation may change depending on your health. Learn more about what to expect during LVAD implantation, including pre-operative instructions, your recovery and follow up.

About the LVAD device

About the LVAD device The job of the LVAD is to help your weakened left ventricle pump blood. Unlike in the past, LVAD devices are now portable. This means you can return home with the LVAD and continue your normal activities while waiting for a heart to become available. In order to receive an LVAD, we need to perform a number of tests to determine that you are a good candidate for the device.

LUNG TRANSPLANT PROCEDURE

A lung transplant involves removal of diseased lung and replacing it with a healthy lung, sourced from deceased donor. Lung transplant is considered as the last resort to survive in the patient with end-stage lung failure and highly diminished survival rates. The transplantation process involves one or two lungs and sometimes combined with the heart transplantation. The procedure is performed under general anesthesia.
A number of respiratory diseases are treated and lung transplantations are offered for various lung disorders at Gleneagles Global Hospitals. They are:

  • End stage lung disease
  • Cystic fibrosis
  • Pulmonary hypertension
  • Pulmonary vasodilators
  • Chronic obstructive pulmonary disease (COPD)
  • Interstitial lung disease

Cystic fibrosis

Cystic fibrosis is a very serious, even fatal, genetic disease which commonly affects children and young adults. In this condition, the lungs and the digestive system get clogged with sticky mucus leading to respiratory and digestive problems. It is a progressive disease which worsens with time.

Causes:

Cystic fibrosis occurs in a child who inherits two defective copies of the cystic fibrosis-associated genes, one gene from each parent. Due to this gene, the cells struggle with the salt and water movement across their walls. This can lead to the accumulation of thick and sticky mucus in the passages when accompanied by infections.

Symptoms

Symptoms can appear soon after the child’s birth or early in the childhood or sometimes may not be seen until adulthood. The first and the most important sign of cystic fibrosis is the salt taste of the skin of the baby when kissed, or the baby does not pass stools as expected soon after birth. The other symptoms appear at later stages of life. Respiratory symptoms:

  • A persistent cough with thick and productive mucus
  • Shortness of breath and wheezing
  • Recurrent chest infections including pneumonia

Treatment

  • Antibiotics to prevent chest infections
  • Medicines which promote the absorption of food
  • Medicines which cause widening of the airways and reduce the inflammation
  • Medicines which cause thinning of the mucus to expel it easily out of the lungs through coughing
  • Techniques and devices to clear off the mucus from the lungs

Gastrointestinal symptoms

  • Weight loss or not gaining weight even after eating in increased quantities. This is due to the inability to efficiently absorb proteins and fats
  • Bowel disturbances including frequent, oily stools or intestinal obstruction
  • Stomach pain and discomfort can occur if too much gas builds up or if constipation occurs

Diagnosis

Most of the cases of cystic fibrosis are detected immediately after the child’s birth using the newborn blood spot test. Tests are also conducted during pregnancy to check if cystic fibrosis is present in the fetus. If your doctor suspects cystic fibrosis, then a sweat test is ordered. If the test result is positive, it indicates that excess levels of salt are present in the sweat compared to the normal. Thus, further tests are conducted on this basis and include one or more of the below:

Test typePurpose of the test
Genetic test To check for the presence of the faulty gene either in the blood or the salivary sample
Chest X-ray Creates images/treatments of the structures in the chest region To check for inflammation in the lungs, scarring and air entrapment
Sinus X-ray To detect sinusitis which is a complication of cystic fibrosis
Lung functioning Measures the breathing ability of the lungs
Sputum culture Presence of Pseudomonas bacterium indicates an advanced stage of cystic fibrosis

Pulmonary fibrosis

Pulmonary fibrosis is the scarring of the deep tissues of the lungs which can develop either slowly or quickly. The tissue gets thick and stiff making it difficult to breathe, reducing the oxygen levels in the blood.

Symptoms

  • Shortness of breath especially while exercising which can become evident even at rest with time
  • A dry and hacking cough which can become uncontrolled over a period
  • The other pulmonary fibrosis symptoms which develop with time include:

  • Fast and shallow breathing
  • Slow, unintended weight loss
  • Fatigue (extreme tiredness)
  • Malaise (feeling unwell)
  • Painful joints and muscles
  • Clubbing of tips of fingers and toes

Causes

  • Cigarette smoking
  • Certain viral infections
  • Environmental pollutants such as hard metal dust, silica, animal proteins, bacteria, gasses, and fumes
  • Gastrointestinal reflux disorder in which the backward movement of acid from the stomach into the lungs can lead to pulmonary fibrosis
  • Medical conditions including tuberculosis, pneumonia, rheumatoid arthritis, scleroderma, etc.

Risk factors

The below factors can make you more prone to getting pulmonary fibrosis

  • Genetics - In some families, at least two people have pulmonary fibrosis
  • Certain medicines such as some antibiotics, heart medicines, and chemotherapeutic drugs
  • Radiation therapies

Complications

If pulmonary fibrosis is not addressed over time it can result in the below complications:

  • Pulmonary hypertension: A serious illness in which the blood pressure in the lungs increases. It progressively worsens and can even prove fatal.
  • Cor pulmonale :This is a condition in which the lower right side of the heart fails to work properly. It can occur due to blockage of the pulmonary arteries.
  • Lung cancer: The risk of you developing lung cancer increases as the pulmonary fibrosis gets advanced and is left untreated for a long time.
  • Respiratory failure : This is the final stage of chronic pulmonary fibrosis.
